ABSTRACT

<p><b>OBJECTIVE</b>To explore the primary site and pathological feature of neuroendocrine neoplasm (NEN), especially the NEN of digestive system.</p><p><b>METHODS</b>Clinicopathological data of NEN patients at China-Japan Friendship Hospital from January 2012 to December 2016 were retrospectively analyzed. Tumor primary sites were summarized. Association between tumor site and pathological grading in gastroenteropancreatic neuroendocrine neoplasm(GEP-NEN) was examined.</p><p><b>RESULTS</b>There were a total of 903 cases of NEN. Sites of primary tumor included the digestive system in 699 cases(77.4%), the thorax(including lung, thymus and mediastinum) in 87 cases(9.6%), other sites in 60 cases (6.6%), unknown in 57 cases(6.3%). Among 699 GEP-NEN cases, the primary sites included the stomachin in 207 cases (29.6%), pancreas in 201 (28.8%), rectumin in 185 (26.5%), duodenum in 43(6.2%), jejunum and ileum in 18(2.6%), appendix in 15 (2.1%), gallbladder in 11(1.6%), esophagus in 10(1.4%), and the colon in 9 cases (1.3%). Pathologically, the tumor grading was neuroendocrine tumor (NET) G1 in 336 cases(48.1%), NET G2 in 203 cases (29.0%), neuroendocrine carcinoma (NEC) G3 in 139 cases (19.9%). All the esophagus NEN(10/10), most gallbladder NEN(9/11) and colon NEN(6/9) were poorly-differentiated NEC (G3), while all appendix NEN(15/15), most stomach NEN(147/207, 71.0%), pancreas NEN (156/201, 77.6%), rectum NEN (169/185, 91.4%), duodenum NEN (31/43, 72.1%), jejunum and ileum NEN(16/18, 88.9%) were well-differentiated NET G1 or G2.</p><p><b>CONCLUSIONS</b>The most common primary site of NEN is the digestive system. The stomach, pancreas and rectum are most common primary sitesof GEP-NEN. Difference in pathological grading is quite greatin different primary sites of GEP-NEN. Most NENs fromesophagus, colon and gallbladder are poorly-differentiated NEC.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study subtype classification of gastric neuroendocrine neoplasm (NEN) and their clinicopathological characteristics in order to provide reference for clinical practice.</p><p><b>METHODS</b>Clinicopathological data of 241 gastric NEN patients (174 cases from China-Japan Friendship Hospital and 67 cases from The First Affiliated Hospital of Sun Yat-Sen University) between January 2011 and June 2016 were retrospectively summarized. According to serum gastrin, 24-hour intragastric pH monitoring and pathological grade, patients with gastric NEN were divided into 4 types: type I( (hypergastrinemia and achlorhydria, related to autoimmune chronic atrophic gastritis), type II( [hypergastrinemia and Zollinger-Ellison syndrome, related to gastrinoma or multiple endocrine neoplasia type I( (MEN-I()], type III( (sporadic disease with normal serum gastrin level), and type IIII( [poorly differentiated gastric neuroendocrine carcinoma (NEC) and mixed adenoneuroendocrine carcinoma (MANEC)]. Clinicopathological features, treatment and prognosis of 4 types were analyzed.</p><p><b>RESULTS</b>Of 241 gastric NEN cases, there were 86 cases (35.7%) in type I(, 7 cases (2.9%) in type II(, 61 cases (25.3%) in type III( and 87 cases(36.1%) in type IIII(. Among 86 cases of type I( gastric NEN, 73 cases (84.9%) were multiple lesions,tumor size of 66 cases (76.7%) was less than 1 cm, all the 86 cases were polypoid or granular lesions. 2 cases(2.3%)presented distant metastasis, 69 cases (80.2%) had pathological grading as NET G1; most of them received endoscopic surgery treatment and follow-up; somatostatin analogs(SSA) was used in patients with multiple lesions and repeated recurrence after endoscopic treatment. Among 7 cases of type II(, 4 cases were gastrinoma, 3 cases MEN-I(; 5 cases presented distant metastasis; treatment included surgery, SSA and proton pump inhibitor (PPI) therapy. Among 61 cases of type III( gastric NEN, 49 cases(80.3%) were single lesion,tumor size of 25 cases(41.0%) was more than 2 cm, 29 cases(47.5%) had lymph node metastasis or distant metastasis; treatment included endoscopic resection, surgery or SSA therapy according to the tumor staging. Among 87 patients of type IIII( gastric NEN, 74 cases(85.0%) had single lesion,tumor size of 51 cases (58.6%) was more than 2 cm; lesions were found in gastric cardia in 35 cases (40.2%); 65 cases (74.7%) had lymph node metastasis or distant metastasis; treatment included chemotherapy, or surgery plus chemotherapy. At the end of follow-up(June 30, 2016), 58 patients were dead, including 1 case of type I(, 12 cases of type III( and 45 cases of type IIII(. The overall survival rate of all the patients was 74.2%, and was 98.8%, 100%, 79.3%, 39.2% of types I(, II(, III(, IIII( respectively. The overall survival rate between type III( and type IIII( gastric NEN was significantly different(P = 0.000).</p><p><b>CONCLUSIONS</b>Subtype classification of gastric NEN is very significant for making therapeutic decisions and prognostic evaluation. Patients of type I( or type II( gastric NEN have good prognosis,while those of type III( and type IIII( have poor prognosis, and those of type IIII( have the worst prognosis.</p>

ABSTRACT

Objective:To investigate the anticaries effects of hesperidin in rats.Methods:The MIC of hesperidin against Streptococ-cus sobrinus(S.sobrinus)was explored with disc diffusion method.The rats with artificial caries were administered with 1/2 MIC hes-peridin,0.12%Chlorhexidine and distilled water respectively.The results of S.sobrinus level,Keyes scoring and DIAGNOdent exam-ination were used to evaluate the effects of hesperidin on S.sobrinus and caries development.Results:The MIC of hesperidin agaist S. sobrinus was 8 mg/ml.The S.sobrinus level was not statistically different between hesperidin group and negative control group(P>0. 05).Keyes scores of grade E of smooth and pit and fissure caries in hesperidin group were lower than in negative control(P<0.05), those of grade Ds and Dm of pit and fissure caries in hesperidin group were lower than in negative control(P<0.05 ),while higher than in chlorhexidine group(P<0.05).Hesperidin showed significant anti-caries effect(P<0.05)examined by DIAGNOdent.Con-clusion:Hesperidin at 1/2 MIC has anti-caries effect in rats without influence of the oral microecological balance.

ABSTRACT

Objective To evaluate extended criteria donor liver in adult cadaveric liver transplant.Methods 126 liver transplantations were performed from January 2003 to June 2009,of them,74 patients received standard criteria donor livers,52 patients received extended criteria donor livers.These 52 donor livers could be divided into two groups:E1 group (a graft with 1 to 2 risk factors) and E2 group(a graft with 3 to 4 risk factors).Results There was no significant difference in half a year and 1 year survival rates between patients received E1 group extended criteria donor livers and those received standard criteria donor livers(respectively x2 =2.55,3.64,all P >0.05).But 2 year survival rate of patients received E1 group extended criteria donor livers was lower than those receiving standard criteria donor livers (x2 =4.9,P <0.05).Half a year,1 year and 2 year survival rates in patients receiving E2 group extended criteria donor livers were less than those receiving standard criteria donor livers (respectively x2 =3.91,8.67,11.34,all P < 0.05).The half a year,1 year,and 2 year survival rates of patients received extended criteria donor livers with MELD score more than 20 was less than those with MELD score < 20 (respectively x2 =0.16,0.16,0.07,all P < 0.05).Conclusions Extended criteria donor livers can be used safely if the risk factor of donor liver was less than 3,or when recipient's MELD score was <20.

ABSTRACT

Objective To analyse the relationship between T lymphocyte subsets and NK cells expression and dynamic changes in lung cancer patients 'peripheral blood and the occurrence and development of cancer,and investigate their clinical significances.Methods Flow cytometry was applied to detect 66 patients with lung cancer,60 patients with pulmonary tuberculosis and 60 healthy persons peripheral blood CD+3,CD+3CD+8,CD+3CD+4,Th/Ts,CD+16CD+56 expression.Lung cancer group peripheral blood CD+3,CD+3CD+8,CD+3CD+4,Th/Ts,CD+16CD+56 expression were also detected on 3rd,7th and 20th day before and after chemotherapy.Results Lung cancer group CD+3,CD+3CD+4,Th/Ts,CD+16CD+56 expression decreased significantly [(54.23±10.37)%,(34.23±8.03)%,1.35±0.20,(25.18±4.34)%] and had significant differences compared with pulmonary tuberculosis group [(63.09±9.19)%,(39.46±12.74)%,1.51±0.41,(26.45±3.96)%] and healthy group [(69.68±8.31)%,(42.31±13.29)%,1.89±0.48,(29.44±2.51)%](P < 0.05),but CD+3CD+8 expression showed no significant difference(P > 0.05).In chemotherapy group,comparing with before chemotherapy,remission group CD+3,CD+3CD+4,Th/Ts and CD+16CD+56 expression decreased significantly (P < 0.01)on 3rd day after chemotherapy,while CD+3CD+8expression increased significantly(P < 0.01).On 7th day,each index recovered to the level of before chemotherapy basically.On 20th day,CD+3,CD+3CD+4,Th/Ts and CD+16CD+56 expression increased significantly(P < 0.05)compared with before chemotherapy,while CD+3CD+8 expression significantly decreased(P < 0.05).Chemotherapy unease group had no significant difference (P > 0.05).Lung cancer of stage Ⅲ A and Ⅲ B compared with stage Ⅰ A,and lymph node metastasis in N3 group compared with N0 group,CD+3,CD+3CD+4,CD+3CD+8,Th/Ts and CD+16CD+56 expression had significant differences (P < 0.05).Compared with their pathological types,each index had no significant difference(P > 0.05).Conclusion Monitoring the peripheral blood T lymphocyte subsets and NK cells dynamic of lung cancer patients can guide the clinical diagnosis and treatment,and contribute to the assessment of immune function.

ABSTRACT

Objective:The roles of HMGA2 in maintaining malignant phenotypes of the osteosarcoma U2OS cells was studied to explore the possibilities for it to be developed as a target for gene therapy.Methods:U2OS cells were stably transfected with a DNA based shRNA expression vector which targeted to HMGA2.The expression of HMGA2 mRNA was proved by RT-PCR;Cell growth,migration and apoptosis were determined with CCK8,hoechst33342 staining and Boyden ventricle,respectively.The mRNA levels of Caspase 3 and Caspase 9 were determined by real time quantitative RT-PCR.Results:The transfection with shRNA expression vector significantly decreased HMGA2 mRNA levels of U2OS cells.Cell growth and migration were decreased,but apoptosis and the mRNA levels of Caspase 3 and Caspas 9 were increased following the decrease of HMGA2 mRNA.Conclusion:The abnormal expression of HMGA2 plays an important role in maintaining the malignant phenotypes of U2OS cells.Gene therapy targeted to HMGA2 could be helpful in the treatment of human osteosarcoma.

ABSTRACT

Objective To investigate the efficacy of preoperative transcatheter artery chemoembolization (TACE) correlates with outcome after orthotopic liver transplantation (OLT) in patients with hepatocellular carcinoma (HCC). Methods Sixty-seven patients with HCC underwent TACE before OLT. In all the patients, there were 52 males and 15 females with age ranging from 34 to 67 years old (average 48). Sixty-one patients met the Milan' standard, and 6 patients exceeded Milan' standard. Classic OLT was done in 62 patients and pigyback liver transplantation was done in 5 patients. Immunosuppression regimen after operation included Tacrolimus (or Ciclosporin,Sirolimus), mycophynolate, steroid hormone. The efficacy of TACE was assessed according to histological findings after OLT. Tumor recurrence rate of 1-and 2-year tumor recurrence rate, and 1-and 2-year survival rate in different groups with different responses to preoperative TACE were compared. Results Tumor necrosis rate was greater than 50% in 50 patients and new tumor nodi occurred in 2 patients. Tumor necrosis rate after TACE was 73. 77%. Tumor necrosis rate was less than 50% in 17 patients and new tumor nodi occurred in 7 patients. Forty-eight patients had satisfactory efficacy after TACE and no new tumor nodi occurred, 1-and 2-year tumor recurrence rate was 2. 08/ (1/48) and 6. 25% (3/48) respectively, and 1- and 2-year survival rate was 97. 92% (47/48) and 95. 83% (46/48) respectively. Nineteen patients had worse efficacy after TACE and new nodi occurred, 1- and 2-year tumor recurrence rate was 36. 84% (7/19) and 57. 89% (11/19), and 1-and 2-year survival rate was 73.68% (14/19) and 47. 37% (9/19) respectively. There were statistically significant difference between the two groups (P<0.05). Conclusion TACE provides good local control in preoperatively diagnosed HCC, but its impact is limited in lesions not detected preoperatively. Patients with satisfactory efficacy after TACE before OLT had a good prognosis. The response to preoperative TACE may predict long-term outcome after LT.

Objective To investigate the biological characteritics of adult animal hepatocytes induced by phenobarbital sodium(PBS) in vivo,and to study the potential value of biological artificial liver of effective hepatocytes.Methods 12 adult male mice,are yandomly divided in to preinducing group and controll group.The preinducing group are intraperitoneally injected PBS per day,45mg/kg for 7 times in total;the controll ware injected NS.after that,we detected the blood serum TP,BUN,CHOL,HDLC.And the same amount of isolative hepatocytes was developed after being developed 48h;MTT was used to investigate the proliferation of hepatocytes after being developed 24h;chromosome was investigated to observe the cell division;and the survival deadline and morphology was also investigated.Results The TP had remarkable difference between the two groups(t=2.678,P

ABSTRACT

Objective To study the effects of ethanol extract from Java Brucea Fruit on platelet-derived growth factor receptor ?(PDGFR?) mediated cell migration and to obtain the valuable messages on the characteristics of its active ingredient.Methods PAE cells was transfected with the vector expressing human PDGFR? with Transfectom 2000;After screening by G418 resisitance,RT-PCR was used to monitor the expression of PDGFR? in the cells;Wound healing of the cells was used to examine the lowest consistency of PDGFBB and inhibitory effect of ethanol extract of Java Brucea Fruit on cell migration after restoring 24 h.Results Human PDGFR? was stably expressed in PAE cells transfected with the expressing vector.The lowest consistency of exogenous PDGFBB which promoted PDGFR? mediated cell migration was 10 ?g?L-1.70% ethanol extract of Java Brucea Fruit which strongly inhibited PDGFR? mediated cell migration was dose-dependent(P10 ?g?L-1) mainly caused the death of the cell.Conclusion Ethanol extract of Java Brucea Fruit has a strongly inhibitory effect on the PDGFR? mediated cell migration which could play a major role in its effects against metastasis of malignant tumor,the active ingredients of it could be more dissolvable in the 70% ethanol.
